Name Change Alert: Home and Community Care Support Services is now Ontario Health atHome.


What do the acronyms CCAC, LHIN and HCCSS have in common? They are the predecessor acronym names of the same government organization coordinating local home and community care and more, in our Ontario community. Effective June 28, 2024 the name of the organization has changed to Ontario Health atHome, but the services provided remain the same.

As per the Ontario Health atHome website they are “... a single organization coordinating local home and community care, long-term care placement and help finding services in the community. Our name has changed but our services remain the same. Patients continue to work with the same patient care teams and can contact us the same way. ”

Some of the services that Ontario Health atHome looks after include:

  • Discharge from the hospital

  • Applying for long-term care

  • Home care services
