Empowering Mississauga’s Seniors: How PCH is Shaping the Future of Elderly Care

Partners Community Health (PCH) is a non-profit organization dedicated to developing a comprehensive healthcare system for the elderly and vulnerable populations in Mississauga and West Toronto. They specialize in long-term care, senior services, and integrated healthcare through their facilities, which include Wellbrook Place. This innovative community includes two long-term care facilities and a Seniors Hub that provides individualized support and holistic care activities.

One of the most recent initiatives of PCH is the Mississauga Health Seniors Navigation Portal - a navigation portal for residents of Mississauga region that connects older adults to healthcare providers. 

The process is simple: 

  1. Complete 5 minute questionnaire

  2. Attend a phone appointment with Care Navigator; 

  3. Be connected to one or more community support services. 

The Mississauga Health Seniors Navigation Portal vision is to improve the health and wellness of all people in Mississauga community by increasing access and connecting care across the system.
